Hi, Peter,

No, the problem is with the drive, not with you.  I ran the HP
StorageWorks diagnostics and got the following output:

    - There are indications of problems reading the tape servo data.
    - However, the drive has not been cleaned since the last occurrence.
    - Please clean the drive with an unexpired cleaning cartridge.
    - Rule 3
    - There was a problem reading the tape servo code.
    - The media could be showing signs of wear.
    - It is advisable to discard this tape (Serial Number: 1140200015).
    - Please use an unexpired cleaning tape, then try using a new cartridge.
    - hp recommends the use of hp storage media.
    - Please run the LTO Drive Assessment Test to check the operation of your
      drive.
    - Device Analysis has checked the historical information and some problems
      were found.
    - There has been an error while writing to tape.
    - Please re-run the test with a different known good tape.
    - Write command failed at block 1041
    - Byte2 0x3 SenseCode 0xc SenseQual 0x0

As advised, I ran the cleaning tape, then loaded your other test tape and ran 
the diagnostics again:

I'll open a trouble ticket with Open Storage -- it's probably time for 
another drive swap.
